What if I am currently on the Premier Plan of benefits and want to continue on the Premier Plan?
All Active members and their legal spouse are required to have a Biometric Screening for 2019, as this is part of the Premier Plan’s Requirement.
Therefore, you must complete this requirement between January 1, 2019, and no later than September 30, 2019, to remain on the Premier Plan of benefits on January 1, 2020.
What if I am not currently on the Premier Plan of benefits and I attend the Medical and Retirement Benefits Meeting?
Because you attended one of our events (member and spouse, if applicable) and complete your Reinforcing Smart Choices Program requirement, we will waive the 90-day waiting period and your benefit coverage will change to the Premier Plan on the 1st of the month following the attended event.
If you are enrolled with Kaiser Permanente, no appointment is necessary to take the in-person class at one of the events listed above. Simply attend one of our events and attend the class being held on the date and time(s) indicated; or, to take the on-line health risk assessment, you must register and be signed on to Kaiser’s website. If you haven’t registered yet, start by going to kp.org/register. Once you have registered on kp.org, go to My Health Manager and choose “My medical record.” Once you’ve completed the online questionnaire, you’ll receive a customized action plan to help you succeed in creating a healthier lifestyle.
If you are enrolled in the Fee For Service Plan, you must register with Quest and make an appointment in advance to complete your Biometric Screening at one of the events listed in this notice or at one of their patient service centers. Please see the instructions below to schedule your appointment.
Instructions for Quest Biometric Screening Registration for FFS (Fee-For-Service) PPO Participants
If you are currently covered under the Fee For Service Plan (Anthem), you may begin the registration process for your biometric screening by going to My.QuestforHealth.com
If you’ve already established an account online with Quest Diagnostics, use the log in area to enter your username and password and select the green Log In button.
If you’ve forgotten your login information, use the forgot your username / passwordlinks to retrieve your username or the password link to reset your password.
If you’ve never registered on the Quest site, to establish an account, use the Create Account area and follow the steps below:
Enter the registration key: CAFieldironworkers and select the Register Now button.
Follow the authentication prompts of: Please enter your Anthem Blue Cross ID (for example; 123456X78), for a spouse member Anthem Blue Cross ID with an S on the end (for example; 123456X78S) and date of birth.
Once your eligibility has been confirmed and your personal information matched, you will be able to select to participate at either one of our onsite events, at a local Quest Patient Service Center, or by a Physician’s results form.

Please note that all appointments for screenings to be performed at one of our onsite events must be completed no later than the date indicated for each meeting and no one will be able to register either online or by phone after the given date. Walk Ins to an onsite event will not be accommodated.
