It is time for Open Enrollment and this year, we have great news; there are no benefit changes for 2018! If you are happy with your current plan of enrollment, there is nothing for you to do and your current benefit elections will continue through 2018.
Should you wish to make changes to your current elections, all of the available Plan options, separated by state, were listed on the back of the cover letter that was included in the package mailed to you from the Trust Fund Office. In the event that you wish to review the complete Plan comparisons, please find the comparison appropriate to your benefits at the bottom of this page. Following review, if you wish to make changes, you may request enrollment packages in one of the following ways:
1. USE THE FORM PROVIDED BY MAIL
Mark your Plan elections on the reverse side of the form provided to you by mail in your Open Enrollment package and mail it to the Trust Fund Office. We will in turn provide you with the requested enrollment packages. Completed applications must be received by the Trust Fund Office no later than December 15, 2017 for an effective date of January 1, 2018.
2. REQUEST BY EMAIL
Request an enrollment package by email at firstname.lastname@example.org. Please be sure to include your name and member id which was indicated on the cover page of the Open Enrollment package you received in the mail, along with the names of the Plan enrollment packages that you wish to receive.We will in turn provide you with the requested enrollment packages by mail. Completed applications must be received by the Trust Fund Office no later than December 15, 2017, for an effective date of January 1, 2018.
